PACE is a community-based health care program created for people 55 and over who require nursing-home-level care but prefer to receive it in their own home. We provide a unique alternative to nursing home care, with comprehensive medical and social services to help your loved one remain at home as long as possible. Our care team includes primary care physicians, nurses, social workers, rehab therapists, home aides and other healthcare professionals who specialize in the needs of seniors. On-call health care coverage by doctors and nurses is available 24 hours a day, 7 days a week, 365 days a year.
