ABOUT ADULT DAY SERVICES Horizon Health Adult Day Services division is part of a non-profit organization committed to providing the highest quality of compassionate and affordable care to dependent adults. Adults we care for may have various forms of dementia or are suffering from a stroke. The Adult Day Services offers professional staff, meals, and activities. Pre-assessments are required to determine a service plan for each individual that is directed at maintaining or improving the participant's capabilities for self-care.
