Jivan Adult Day Health Center is a program which provides health care services to elders, including daily registered nursing care, chronic disease management, ongoing medical and preventative health care, case management, family support, and a hot meal and snacks. The center will provide the members with great care and the chance to mingle and be with peers and to participate in activities outside the home. The center will also offer weekly trips & excursions, picnics (weather permitting), shopping trips. Languages services available in Cape Verdean Creole, Hindi, Gujarati, Spanish, Portuguese, and English. A place filled with Love and Happiness! Call us for a tour!
