Senior CommUnity Care of Kentucky (PACE) Program of All Inclusive Care for the Elderly is designed to provide comprehensive and coordinated care to older adults who meet certain eligibility criteria. The goal of PACE is to support individuals to remain in their communities and live as independently as possible by offering a range of medical, social, and emotional support services. PACE is a unique model of care that includes a team of healthcare professionals working together to create personalized care plans for each participant.
