Welcome to Autumn Leaves Adult Day Center Located on the East Indianapolis, Autumn Leaves Adult Day Center provides engaging activities, meaningful socialization, and essential health services for older adults and adults with disabilities—all within a beautiful and safe environment. We are open Monday-Friday 7am-6pm, and the 1st and 3rd Saturday over every month 8am -4pm. We are a certified Medicaid provider. Our faith-based, person-centered program is designed to meet each individual's needs and is open and welcoming to all, regardless of religious background or beliefs.
